If you are considering surrendering your horse, please read this information first.
Download and print this form if you intend to donate or surrender your horse to Amazing Grace Equine Sanctuary. Please make sure to call us at 262.627.0582, or email Erin@rescuehorses.org, about your horse before sending us this form to ensure that we have available space and can meet your horse's needs. After you have filled out the form, you may scan it, and e-mail it back to AGES.

If you would like to be approved to adopt a horse from Amazing Grace Equine Sanctuary, print this form and fill it out. You do not need to have a specific horse in mind when you submit your application and applying does not obligate you to adopt a horse. After you have filled out the form, you can mail it to AGES or scan it and e-mail it to AGES.
